SAFETY EVALUATION REGARDING MODIFICATION OF THE ALTERNATE MINI-FLOW SYSTEM FOR CHARGING/SAFETY INJECTION PUMPS -
SHEARON HARRIS POWER PLANT, UNIT 1

During the week of August 8,
- 1992, an NRC inspection was conducted to review the circumstances pertaining to a reported loss of the plant high-head safety injection (HHSI) that occurred on April 3, 1991, at the Shearon Harris Nuclear Power Plant (SHNPP).
This inspection (Inspection Report 50-400/92-201 issued on September 1,
1992) raised operability concerns for the SHNPP HHSI's alternate mini-flow (AHF) system.
In a letter dated August 14,
- 1992, NRC formally expressed these concerns to Carolina Power
& Light Company (CP&L) and met with the licensee on August 20, 1992, to discuss the operability of the HHSI ANF system.
By a letter dated September 8,
- 1992, CP&L provided a written response to the staff concerns and indicated that a design change to eliminate relief valves in the AMF system for the HHSI charging/safety injection (C/SI) pumps at the SHNNP was also being considered.
On September 15,
- 1992, CP&L indicated in a phone call to the staff, that the ANF system will be modified during the current refueling outage
- and, on October 7,
- 1992, met with the staff to further discuss details of the ANF system modification.
As a result of the October 7, 1992, meeting, CP&L submitted specific details of the modification by letters dated October 9, and October 12,
- 1992, as supplemented by letters dated October 28, and November 5, 1992.
The modification would remove spring-loaded pressure relief valves from the AHF lines and 'replace them with 60 gallon-per-minute flow control orifices and add automated, logic.to control the opening and closing of motor operated isolation valves in:the lines.
The staff reviewed CP&L submittals and finds the modification to the AHF system for the HHSI's C/SI pumps to be acceptable, contingent upon the satisfactory completion of post-modification testing of the four motor-operated valves (HOVs) in the modified ANF system and verification and confirmation of assumptions as discussed in the enclosed safety evaluation.

CP&L's implementation of testing and verification and confirmation of assumptions as described in the SE should be documented and records maintained for possible future NRC audit.
