This refers to the Nuclear Regulatory

Commission

(NRC) administered

requal-

fication examinations

conducted

by C.

Rapp during the weeks of July 15 and 22,

1991.

These

examinations

were administered

to employees

of your company

who

currently hold licenses

to operate

the Shearon

Harris Nuclear Plant.

At the

conclusion of the examination,

the examination questions

and preliminary

findings were discussed

with those

members of your staff identified in

Enclosure

1.

A copy of the written examination

questions

and answer

key was given to

members of your training staff.

A Requalification

Program Evaluation Report

is included

as Enclosure'.

This evaluation report finds the Shearon Harris-

Nuclear Plant requalification program remains satisfactory

based

on the

evaluation criteria of NUREG-1021, Operator Licensing

Examiner

Standards.

Any

identified operator or programmatic

weaknesses

are included in this

evaluation.

These

weaknesses

were discussed

with members of your training

staff.

A Simulation Facility Fidelity Report is included

as Enclosure

3.

During the simulator portion of the operating

examinations,

the operators

initiated

a manual

safety injection before assuring

the reactor

was shutdown.

If the reactor failed to shutdown,

an anticipated transient without scram with

inadequate

heat

removal could result.

This condition has

been

recognized to

be

a significant precursor to core melt scenarios.

You are invi'ted to respond

to this office, within 30 days after receipt of this report,

as to corrective

actions for this condition.

In accordance

with 10 CFR 2.790(a) of the Commission's

Regulations,

a copy of

this letter and the enclosures will be placed in the NRC'ublic Document

Room.
