50-400/89-15 Violation "A" Re orted Violation:

10 CFR 55.53(e) requires a

Senior Operator to perform the functions of a

Senior Operator for a

minimum of five 12-hour shifts per calendar quarter in order to maintain an active license.

10 CFR 55.53(f) states that if part (e) is not met, the facility licensee shall certify the qualifications and status of the licensee as

current, and that the licensee has completed a

minimum of 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> of shift functions under the direction of a Senior Operator prior to resuming license duties.

Contrary to the above, during the second quarter of 1989, a Senior Operator performed the functions of a Senior Operator for four and one"third 12-hour shifts, instead of the required five 12-hour shifts.

The Senior Operator subsequently resumed license duties for three consecutive days in the third quarter of 1989, without obtaining certification as required by 10 CFR 55.53(f).

This is a Severity Level IV violation (Supplement I).

Denial or Admission and Reason for the Violation'.

The violation is correct as stated.

The method used to track hours worked by the license holder did not provide timely information in order to determine that the minimum number of hours were not met.

The method involved a

periodic review of data during each quarter.

The second quarter 1989 ended on Friday, June 30, 1989'he individual responsible for performing this review was off July 1, 2, and 3, and returned on July 4 at which time the review was performed.

By this time, the SRO had worked July 1, 2, and 3 without obtaining proper certification.

Co'rrective Ste s Taken and Results Achieved:

The subject Senior Operator was returned to active status in accordance with the requirements of TI-200, Licensed Operator Requalification Program.

A review of hours worked by other license holders was performed to determine if additional tracking problems existed.

No other instances of insufficient hours worked were found.

Corrective Ste s Tike"t'o Prevent Further Violations'.

The method of tracking hours worked by each license holder has been revised to provide an update on a more frequent bases.

This provides an easier method of ensuring that the proper number of hours have been worked.

Appropriate personnel have been trained on the updated method.
