In accordance with your letter of December 21, 1988, transmitting Inspection Report No. 50-400/88-39, the following is our response to your concerns about the October 30, 1988, plant. trip.

The major contributors to plant trips at Harris since commercial operation have been initiated by the Feed and Condensate System.

Recognizing this, a Feed and Condensate Task Force was established March

1986, completing their efforts in September 1987.

Significant improvements were made as evidenced by a reduction in the number of trips from 14 in 1987 down to 3 in 1988.

However, the trip on October 30, 1988, highlighted to plant management that personnel were not fully cognizant of all the changes made to the systems and that there are still specific plant responses that were not anticipated.

Therefore, following the Plant Nuclear Safety Committee (PNSC) discussion of the LER another Task Force was esta'blished on November 29,

1988, to reassess previous
efforts, and to ensure all programmatic items in this trip are identified.

The Task Force has completed its efforts and concluded that the previous recommendations have been largely implemented.

However, the numerous changes that have occurred has cau'sed some confusion on the part of the operators as to expected plant response.

Recognizing this programmatic issue and the specific problems brought up by this last

trip, the Task Force initiated the following corrective actions'.

Implement the policy of keeping the system descriptions current,.using the advance change process if necessary.

In the case of SD-134, Feed and Condensate, complete update by January 31, 1989.

~

Provide retraining to operators during the currently scheduled requalification cycle.

This is scheduled to be conducted during the simulator portion which will be done by April 15, 1989.

~

Provide operator aides to the auxiliary operators in the form of adding labels to the heater drain sight glasses and providing local equipment pre-start checkoff sheets.

The labels are planned to be added by March 31, 1989.

The pre-start checkoffs are being added to plant operating procedures (beyond just feed and condensate),,

as the procedures become due for periodic revision, or an advance change takes

place, whichever is earlier.

This should be completed by June 30, 1990.

The reset band for the condensate booster pump discharge pressure switch is being corrected by.installation of a

switch of a different design.

This will be completed during the next refueling outge.

~

Reassess the design basis for runback at 1900 MWe per minute.

If possible, reduce runback to a

level.

around 450 MWe/min which allows for better plant response.

Schedule completion for evaluation is June 30, 1989.

Operations will reevaluate the current practice of shifting main feed regulating valves to manual when a transient of this type occurs.

Evaluation is expected to be completed by June 30, 1989.

~

Evaluate under what conditions AFW should be initiated while at power, in an attempt to restore level to steam generators and provide appropriate guidance to operations.

Expected to be completed by June 30, 1989.

,Other specific actions were taken as a

direct result of the October 30, 1988, trip and are described in our LER 88-032-00.

We feel that at the conclusion of these

actions, the problems with feed and condensate should be resolved.

Management shares with the NRC its concern about the programmatic issue of operator awareness of system response.

While the numerous changes made to the system during startup and after initial license training for the operators caused confusion over expected system

response, we feel this action plan will address those deficiencies and minimize a

recurrence.

Also, we do not anticipate such a

number of design changes affecting operator expected response from plant systems to occur now that plant operating stability has been achieved.

A full copy of the Task Force Report will be available on-site for review by NRC inspection personnel.
