This refers to the Nuclear Regulatory
Commission
(NRC) inspection
conducted
by
W.
H. Bradford
on October
21
November
20,
1988,
The inspection
included
a
review of activities authorized for your Harris facility. At the conclusion of
the inspection,
the findings were discussed
with those
members
of your staff
identified in the enclosed
Inspection
Report.
Areas
examined
during
the
inspection
are identified in the report.
Within
these
areas,
the inspection
consisted
of selective
examinations of procedures
and
representative
records,
interviews with personnel,
and
observation
of
activities in progress.
Within
the
scope
of
the
inspection,
no violations
or deviations
were
identified.
As discussed
during the exit interview,
and described
in detail in paragraph
8
of the enclosed
inspection
report,
we have concerns
over the 'operator errors,
secondary
plant
instrument
calibration
problems,
and
apparent
weakness
in
operator
knowledge of secondary
plant equipment that were displayed during the
October 30,
1988 reactor trip event.
It is our understanding
that you
have
formed
a task force to review this event
and
recommend
appropriate corrective
'ctions.
Accordingly, you are
requested,
within thirty days of the date of
this letter, to inform us of the actions
you have taken or plan to take with
respect
to our concerns
and the findings of your task force.
In accordance
with Section
2.790 of the
NRC's
"Rules of Practice,"
Part 2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and its enclosure
will be placed in the
NRC Public Document
Room.
