50-400 88-27 Violation Re orted Violation:
- 10CFR50, Appendix B, Criterion XVI requires that the cause of a
significant condition adverse to quality shall be determined and corrective action taken to preclude repetition..
Contrary to the above, the corrective action taken by the licensee to resolve nonconformance report No.87-098, dated August 7,
- 1987, significant condition adverse to quality, was not adequate to prevent other EQ equipment from being degraded as a result of maintenance.
In particular, on a't least two occasions (September 17, 1987 and April 15, 1988),
the licensee failed to install functional T-drains i.n the motor end bell for limitorque valve operator 1MS-70.'he T-drains were required to restore the valve operator to its tested configuration.
This is a Severity Level IV violation (Supplement I).
'Denial or Admission and Reason for the Violation.'he violation is correct as stated.

Upon review of the circumstances leading up to the'iolation and the status of, the site EQ program, it was concluded that the reason for the violation was a lack of. detailed training in EQ for craft personnel and maintenance planners.

4 Corrective Ste s Taken and Results Achieved:
T-drains were properly installed in limitorque valve operator 1MS-70 and EQ qualification was restored.
Corrective Ste s Taken to Avoid Further Violations:
To avoid further violations, actions have been initiated to expand Training Course CT1010H which is a familiarization in EQ giv'en to craft personnel.
Additional technical information will be included in this course with site specific examples of EQ components and what actions must be taken to ensure that qualification is maintained.
Attendance will be required of craft personnel that perform maintenance on EQ components,
- planners, and selected engineering personnel.
Maintenance procedures for each type of EQ component will be reviewed to determine if corrections to current maintenance procedures are required.
This review will also determine if there is a need for additional procedures.

In the,interim, appropriate Technical Support personnel will be responsible for reviewing ZQ work tickets and will assign hold points for critical activities as deemed necessary.
This process began on November 23, 1988.
When appropriate training of personnel is completed this interim measure will no longer be required.
Date When Full Com liance will be Achieved:
Full compliance will be achieved upon completion of development and implementation of the training course and procedure reviews as stated above.
This is expected to be complete by'April 30, 1989.
