ENCLOSURE 1
NOTICE OF VIOLATION Carolina Power and Light Company Shearon Harris Unit 1 Docket No. 50-400 License No. NPF-63 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 20-August 28, 1988, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:
Technical Specification 6.8.1 requires that written procedures be established and implemented covering procedures outlined in Appendix A of Regulatory Guide 1.33, Rev. 2, February 1978.
Surveillance and Cal.ibratlon procedures are identified -in -Appendix A of the Regulatory Guide.

A surveillance procedure, Operational Surveillance Procedure (OST)-1826, Train "B" SI ESF Response Time Test, requires the lifting of leads in order to test the auxiliary feed water (AFW) pumps.
Contrary to the above, OST-1826 was established inadequately, in that the procedure did not require the main feedwater pump switches be placed in the "Stop" position prior to reconnecting the AFM lifted
- leads, which resulted in an inadvertent start of the "B" train AFM pump on August 9, 1988.

A calibration procedure, Maintenance Surveillance Test (MST)-I-0401, Containment Hi Range Accident Radiation Monitor RM-01CR-3589-SA Calibration, requires calibration data base changes to be entered into the containment Hi Range Accident Radiation
- Monitor, RM-01CR-3589-SA.
Contrary to the above, MST-I-0401 was not implemented, in that on August 8,
- 1988, calibration data base changes for monitor RM-01CR-3589-SA were inadvertently entered into the Control Room Area and Radiation Monitor RC-21RR-3560-SA.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:

- 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Harris, within 30 days of the date of the letter transmitting this Notice.

- achieved, (4) the cor rective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Mhere good cause is
- shown, consideration will be given to extending the response time.
Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
