Each of these events was-initiated by a failure.of the outer door followed by unauthorized use. of the inner door.

The applicable LERs and response to violation 50-400/87-21 illustrate CPSL's corrective actions.

These corrective actions addressed both the reliability of the door and the administrative control over opening the door.

In summary, the corrective actions included:

LER 87-034-00 and Violation 50-400/87-21 (June 12 1987 incident) 1.

The Seals on the inner and outer personnel air lock doors were replaced.

2.

Sign was posted on the outer door as a reminder to notify the Main Control Room prior to entry.

This action was first initiated, although not'ade a formal commitment, in response to LER 87-034. Action was initiated during the July 8, 1987 Plant Nuclear Safety Committee (PNSC) meeting to begin training personnel to operate the air lock during high traffic periods. This action was not immediately implemented. Subsequently it became necessary to require that only trained personnel be allowed to operate the door at any time. This was subsequently documented in Plant Special Order 87"003 dated October 23, 1987. Since September 17, 1987 there have been no further events involving violation of TS 3.6.1.3. In

sumsary, the final corrective actions should have been implemented after the first event.

Based on individual review of LERs in this area and considering the timing of the violations, it is. not felt that the events constitute

a. repetitive pattern.

CPhL considers the corrective actions taken as a result. of these. violations 'to have been adequate. In

summary, CP&L finds that the failure to take adequate corrective action on the three Technical Specification violations concerning the air lock is an isolated event.

Review of plant events for appropriate corrective action is an active, ongoing process for events reportable to the NRC as well as those not reportable to the NRC.
