The

issue
from Inspection
Report 50-400/87-37

letter
is contained
in violation
A of the enclosed

report documenting that inspection
was sent to you by letter dated
November 10,
1987.
The
remaining
issues
addressed
in this letter pertain to Inspection
Report
50-400/87-40.
Inspection
Report
50-400/87-40
included
a
review of
activities
authorized
for your Harris facility.
At the conclusion of this
inspection,
the
findings
were
discussed
with those
members
of your staff
identified in the enclosed
inspection report.
Areas
examined
during this inspection
are identified in the report.
Within
these
areas,
the inspection
consisted
of selective
examinations
of procedures
and
representative
records,
interviews with personnel,
and
observation
of
activities in progress.
The inspection
findings indicate that certain activities
appeared
to violate
NRC requirements.
Based
on additional information discussed
at the enforcement
conference
conducted
with your staff
on November 20, 1987, violations
A and
B
in the enclosed
Notice of Violation have
been
categorized
at Severity Level
IV.
The violations,
references
to pertinent requirements,
and elements
to be
included in your response
are
described
in the enclosed
In addition to the enclosed
violations,
we are also concerned
over your valve
operability
determinations
during the
events
of February
and October
1987
involving reactor
coolant
system
vent
(RCSV) valve spurious
actuations.
Our
position is that the
RSCV valves that were pressurized
open during these
events
should,
in fact,
have
been
declared
As such,
we are requesting
that
you include in your
response
an evaluation of your process
for making
and identify any appropriate
changes
you have
made
or will make to strengthen this process.

Inspection
Report,
paragraph
3 is
similar to
a violation contained in the Notice sent to you by our letter dated
September
8,
1987.
'Because
"similar violations,"
as
described
in the
NRC
Enforcement
Policy,
are
of significant
concern
to the
NRC,
please
give
particular attention
in your response
to the identification of the root cause
of this problem and your corrective action to prevent recurrence.
In accordance
with Section
2. 790 of the
NRC's
"Rules of Practice,"
Part
2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and its enclosures
will be placed in the
NRC Public Document
Room.
