10CFR50.72 requires that.

the licensee notify the NRC Operations Center via the Emergency Notification System when any event.or condition results in manual or automatic actuation of any engineered safety feature (ESF)..

The event is required to be reported within four hours of its occurrence.

Contrary to the above, on August 5,

1987, the licensee failed to report an automatic actuation of the auxiliary feedwater system within the required four-hour time period.

The actuation of this ESF system occurred at 2:01 a.m.

and was not reported to the NRC Operations Center until 9:44 a.m.

This is a Severity Level V violation (Supplement 1).

Denial or Admission and Reason for The Violation'.

The violation is correct as stated.

At the time of the incident the plant was stable in Mode 3, Hot

Standby, at 0 percent reactor power.

Main Feedwater (MFW) pump 1B was in service supplying feedwater to the steam generators.

Previously on August 4,

1987 two attempts to start MFW pump 1A resulted in the pump to trip on high discharge pressure.

At 1:56 a.m.

on August 5, 1987, it was decided to stop MFW pump 1B and try restarting MFW pump lA.

At 2:01 a.m.,

an attempt was made to start MFW pump lA with the discharge valves partially open instead of closed as required by pump logic.

MFW pump 1A tripped, which caused both motor driven auxiliary feedwater (AFW) pumps to actuate as required.

When the MFW pump 1A failed to start, operations personnel were expecting the AFW pumps to start.

The event was not reported within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> as required by 10CFR50.72 due to operations personnels failure to promptly recognize the AFW start as a reportable ESF actuation.

Corrective Ste s Taken and Results Achieved:

Upon recognition that the AFW start was reportable per 10CFR50.72 the required notification was made to the NRC at 9:44 a.m. noting the failure to make the report in a timely manner.
