ENCLOSURE 1
NOTICE OF VIOLATION Carolina Power and Light Company Harris Docket No. 50-400 License No.
CPPR-158 During the Nuclear Regulatory Commission (NRC) inspection conducted on June 9-13,
- 1986, a violation of NRC requirements was identified.
The violation involved failure to have adequate corrective action.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1985), the violation is listed below:
10 CFR 50.54(a)(l) requires Carolina Power and Light Company (CPL) to implement the guality Assurance (gA) Program described or referenced in the Safety Analysis Report.
Section 17.2.16 of the Harris FSAR requires that corrective action to be initiated to preclude recurrence of significant conditions adverse to quality.
Nonconformance report 85-2000 established corrective actions to prevent repeated damage to instrument.tubing in the containment.
Contrary to the
- above, CP8Ls corrective action was inadequate in that on June 11, 1986 four instrument Tubes were found to be damaged.
This is a Severity Level V violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
(1) admission or denial of the violation, (2) the reason for the violation if
- admitted, (3) the corrective steps which have been taken and the results
- achieved, (4) the corrective steps which will be taken to avoid further viola-
- tions, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
