SPR025 1.0 PURPOSE This procedure establishes specific instructions for identification, marking and record management of hoisting and rigging apparatus used for general and special lifts of materials and equipment. These instructions are supplementary to the requirements of MMM-20.

NUREG 0612 Control of Heavy Loads 3.0 RESPONSIBILITIES 3.1 Mana er Maintenance The Manager Maintenance has the overall responsibility for the safe handling of all equipment and material in accordance with referenced specifications and regulations, and for the safe logistical organization of all rigging and handling operations.
3.2 Maintenance En ineer S ecialist The Maintenance Engineer/Specialist is responsible for the following:
Collect and maintain copies of reports pertaining to hoisting equipment and rigging used for general lifts.
These reports shall be made during load tests, testing and special visual examinations. Cranes and forklifts shall be identified by CPSL I.D. number in these reports. He will additionally assign unique identification numbers as required.
Transmit to Quality Assurance, when collected as a result of requirements of procedure, code, standard, specification or purchase order the following reports:
- a. Special visual inspections
- b. Magnetic particle examination or other Non-Destructive Examinations reports.

- 3. Determine the safety factor of rigging arrangements when there may be questions concerning safety requirements.
- 4. Ensure required certification is maintained for all slings, chokers, shackles, cranes and forklifts.
- 5. Identify rigging and hoisting equipment, ensuring that NDE inspections are current and correctly marked in accordance with Paragraph 5 of this procedure.
- 6. Maintain a copy of equipment inspection records.
- 7. Verify the load capacity of all slings and shackles by:
- a. Manufacturer's certifications of load tests
- b. Safe working load ratings permanently affixed to hoisting equipment or rigging
- c. Establishing load test of the lifting device if none of the above is available.
- 8. Ensure that initial NDE is performed on slings or shackles by qualified NDE testing personnel.
- 9. Notify Quality Assurance of all load or rerating tests.
- 10. Review supply requisitions for slings, chokers and shackles to ensure that all appropriate OSHA requirements are included. The requisitions shall set forth the certification and documentation requirements to be included in the purchase order.
3.3 Maintenance Foreman The designated maintenance foreman will ensure that lifting arrangements meet all applicable safety regulations. The foreman will be responsible for required inspection of general rigging hardware and properly identifying all hardware passing inspection documenting same on Attachments 1 through 4.
3.4 Lead Mechanic The Lead Mechanic designated by the Maintenance Foreman shall:
Organize crews and delegate work as detailed or required to assure the safety of personnel, equipment and materials.
Thoroughly inspect all lifting hardware and rigging arrangements prior to making a lift.

Ensure that all slings have an up-to-date or current inspection marker.
Delegate one signal man for each lift, although the signal man need not be restricted from performing other duties concurrently.
4.0 DEFINITIONS 4.1 Hoistin E ui ment Lift crane including the sheave block and hooks (or sheave block shackle) and forklifts.
Fork lift type hoists.
4.2 ~Ri ~in Any sling, choker, shackle or other apparatus used to attach the load to the hoist hook or shackle.
5.0 PROCEDURE 5.1 E ui ment Identification The CPSL equipment number of the cranes, forklifts, slings, or shackles, etc. will be used for identification purpose. The serial number of the block with hook or shackle shall be used for identification purposes of those components. If the hook or shackle does not have a serial number, it shall be marked with a number consisting of the unique I.D. number and safe working load in tons e.g., (XXX-200) when the XXX is the unique I.D. number and 200 being the safe working load in tons (Reference 2.2).
5.2 Certification of Load Ca acities
- 1. Certification of load capacities for hoisting equipment may be verified by Manufacturer's load tables, which are permanently attached to the crane, or available at the crane. Tables can be verified through the manufacturer's serial number and to verify the load capacity of the crane.
- 2. Certification for equipment and rigging shall be retained for the lifetime of the particular item while it is installed in the plant or stored for future use.

SPR025 5.3 Identification of Ri in s Slin s and Chokers The Maintenance Engineer/Specialist shall ensure that all riggings, slings and chokers used for general lifts are collected and inspected at intervals no greater than once every 12 months. The color code identification of rigging shall be as follows:
- 1. Slings and chokers passing inspection shall be marked with a blue or green color coded marker corresponding to the period the inspection is to cover. Period color codes shall be as follows:
Odd calendar years = Green Even calendar years = Blue i.e., 1984 = Blue 1985 = Green 1986 = Blue 1987 = Green
- 2. All hooks, slings and shackles which do not pass the rigging visual inspection shall have a red dot painted on them. The deficiency shall be further investigated by one or more of the following methods:
- a. Magnetic particle testing or other suitable crack detection testing.
- b. Load testing.
- 3. If any of these methods prove the item to be satisfactory, a red dot shall be painted over with blue or green paint as applicable. No shackle or sling having a red dot painted on it shall be used for any type of lift.
Any shackle, pin, or hook shown to have a discontinuity shall be identified by painting a red dot on it. If the hook, shackle. or pin already has a blue or green mark, then the red dot shall be painted on top of the blue or green mark. The discontinuities shall be carefully evaluated and determinations made as to whether further testing or rerating at a lower capacity may be in order.
If either of these methods prove the items to be satisfactory, then the red dots shall be painted over with the applicable color code paint. No sheave block hook, shackle or pin shall be used if it has a red dot painted on it.
- 5. If the item remains suspect because of discontinuities and rerating is judged inappropriate by the Manager Maintenance or his designee, it shall be destroyed by cutting it into pieces.
