~ w DEFICIENCIES IN THE PROCUREMENT, FABRICATION, AND INSPECTION OF ENGINEERED PLATES FOR CONCRETE EMBEDMENT AND SUPPORT OF SAFETY-RELATED LOADS

Dear Mr. O'Reilly:

In accordance with 10CFR50.55(e}, the Region II office (Mr. J. Bryant) was officiallynotified of the subject deficiency as being a "potentially reportable" item on July 6, 1979.

On August 6, 1979, the Region XX office (Mr. R. Bradley} was officiallynotified that the subject deficiency was considered reportable under the provisions of 10CFR50.55(e).

Due to the nature of the problem, all investigation/corrective action has not been completed Attached is an interim report which describes the problem and the corrective action taken to date.

The final report will be submitted by November 2, 1979.
