This will be the last monthly status report on Turkey Point.
Future plant status will be covered under the Region's quarterly Plant Status Report.

ENCLOSURE TURKEY POINT MONTHLY STATUS REPORT (August 1 - September 15, 1988) 1.

Facilit 0 erations Summar Unit 3 is operating at 100K power and has been on line since March 30, 1988 (170 days).
This run surpasses the previous operating records of 122 days for Unit 3 set on September 11,
- 1983, and 130 days for Unit 4 set on October 25, 1980.
Unit 4 is presently operating at 100K power following recovery from a
dropped rod and subsequent turbine runback on September 5,
1988.
The unit was previously off the line on two occasions during this period.
Once from August 12-17,
- 1988, to repair a leak on the pressurizer spray line and also from September 19-22,
- 1988, due to a reactor trip on low steam generator level caused by personnel error.

Items of Interest FPL submitted its response to the Independent Management Appraisal on August 15,
- 1988, and it is currently under NRC review.
On August 1, 1988, with Units 3 and 4 in Mode 1, the licensee reported a
significant event in accordance with 10 CFR 73.71.
This was the result of a security guard leaving a post without permission or full knowledge that the alarm function for the gate was operable.
This resulted in compensatory measures not being met for approximately four minutes.
On August 12,
- 1988, with Unit 4
being shutdown to fix a
leaking pressurizer spray line, the reactor tripped due to source range nuclear instrument (N-32) spiking high.
The reactor was subcritical at the time and all rods were in the core except shutdown bank 8
( 150 steps).
-The licensee reported the significant event in accordance with 10 CFR 50.72.
ISC repaired N-32 and the Unit was returned to service on August 17, 1988.
On August 14, 1988, with Unit 4 in Mode 3, at 11: 14 a.m.,
a partial Train 1,
Phase A, actuation occurred when closing the access door to rack 44.
The actuation was caused by a wire bundle near the door bumping one of the relays in the rack when the door was closed.
Enclosure On August 14,
- 1988, with Unit 3 in Mode 1
and Unit 4 in Mode 3, the licensee reported a significant event in accordance with 10 CFR 50.72.
The licensee lost all offsite hardline telephone circuits due to damage from lightning and rain.
The licensee maintained offsite communication with a portable cellular telephone.
Repairs were completed on August 15,
- 1988, and the hardlines were returned to service.
On August 16,
- 1988, with Unit 4 in Mode 3, the licensee declared an Unusual Event due to a spill of radioactive water from the Unit 4 Spent Fuel Pool (SFP).
The Unusual Event was terminated at 6:00 p.m. that same day.
This event was caused by the vibrating open of the spent fuel pool cooling pump 4A vent valve.
The valve vibrated open during the failure of the pump due to lack of oil.
Subsequent testing of the valve by an independent test group proved the valve would vibrate open't various frequencies.
This event is discussed in detail in inspection report 50-250, 251/88-25.
On August 16,
- 1988, with Unit 4 in Mode 3, the licensee reported a
significant event in accordance with 10 CFR 50.72.
The "A" steam generator feedwater pump (SGFP) tripped on low suction pressure (below 200 psi) which initiated auxiliary feedwater (AFW).
The cause was attributed to a failure of a pressure differential switch (PDIS) 6331B.
The switch was repaired and the Unit was returned to service later that night.

On August 19, 1988, at 9:21 p.m. Unit 4 scrammed from 100% reactor power due to a
"B" steam generator low-low level trip signal.
Technicians were performing Operating Procedure (OP) 14004. 1, Steam Generator Protection Channels-Periodic Test, at the time of the trip.
The auxiliary feedwater system initiated and performed as designed.
The licensee's post trip review identified that the cause of the trip was personnel error.
On August 22,
- 1988, the licensee notified the NRC per 10 CFR 50;72 of a procedural deficiency that directed that the reactor water storage tank (RWST) outlet valves (862A.and 862B) be cycled closed monthly.
These valves are normally maintained open by keeping their circuit breakers locked open in the motor control centers as directed by TS 3.4. 1.a.7.
If the above valves were closed for testing, you would lose the flow path from the RWST for. the time it takes to.cycle the valves, which is approximately one minute.
Also, if a valve failed in the closed position, it would take even longer to restore the flow path from the RWST to the RHR pumps, the containment spray pumps and the high head safety injection pumps.
On August 31,
- 1988, the licensee notified the NRC of a significant event per 10 CFR 50.72 due to the control room ventilation system automatically tripping to the recirculation -mode.
The actuation. occurred while performing a check of the ventilation inlet r'adiation monitor:
The vendor was called in for troubleshooting and initial indication points to a
circuit design problem.

50-250/88-14 Reactor Coolant System Leakrate Surveillance Not Performed Within Required Interval Due to Personnel Error.
Event date - July 14, 1988 50-250/88-15 Post Accident Hydrogen Monitor System Deficiencies Due-to Procedure and Administrative Control Weakness.
Event date - July 29, 1988 50-251/88-06 Containment Ventilation and Control Room Ventilation System Isolation Due to Containment Radiation Levels Exceeding Containment Radiation Monitor Setpoint.
Event date - July 16, 1988 Unit 4 is scheduled to conduct a refueling outage beginning September 18, 1988 and ending December 22, 1988.
On August 30,
- 1988, the.-licensee announced the following organizational changes in the newly established nuclear engineering organization:

Mr. D. A. Chancy was promoted to the position of Director - Nuclear Licensing A, meeting in Headquarters was held on August 18,'988, with FPL representative regarding the Technical Specification conversion project.
FPL will submit comments to the "Proof and Review" Technical Specification by September 23, 1988.
On August 10,

. In a letter dated August 15, 1988, the licensee provided a.progress summary of the Select System assessment.
The design basis verification activities for the original fifteen systems. continues with a completion scheduled for May 1989.

NRC Staff Activities Durin the Period The SALP Board met on August 23,
- 1988, to evaluate Turkey Point's performance for the period. June 1,
1987 to June 30, 1988.
The SALP report was. signed on September 13, 1988.
The following ratings were assigned:

The resident inspector's monthly report covered the period July 26-August 25, 1988.

One finding was identified for failure to maintain compensatory measures.
