reviewed your implementation of
a program
as required by
10 CFR 50.49 for establishing
and maintaining the
environmental qualification (Eg) of electrical
equipment within the scope of

areas,
the inspection consisted of review of
selected
procedures
and records,
examination of selected
plant equipment,
interviews of personnel,
and other observations
by the inspectors.
The inspectors
determined
thai: you have
implemented
a program to meet the
requirements
of 10 CFR 50.49 except for certain deficiencies identified in the
enclosed
inspection report.
Five of these deficiencies,
summarized
in
Appendix A, are classified
as Potential
Enforcement/Unresolved
Items
and will
be referred to the
NRC Region II Office for further action.
These
items
related to Raychem shrinkfit sleeve insulation in unqualified configurations,
failure to show similarity between tested
3N tape splice insulation
and that
installed
and potentially improper level of qualification, operation of the
plant between
March and October of 1985 with unqualified Texaco Marfak grease
in Limitorque motorized valve actuators,
and lack of traceability to Eg
documentation
of two field-run cables to Limitorque valve actuators
due to
improper cable tag
numbers
and absence
of other identifying cable markings
within the the actuator.

a result of the equipment qualification inspection
performed during the
period of Harch 2-6,
1987 at the Florida Power
and Light Company's
(FPEL) Juno
Beach engineering offices and the Turkey Point nuclear Generating Station
(PTN),
the following items
have
been referred to the
NRC Region II office as Potential
Enforcement/Unresolved
Items.

Contrary to paragraphs
(f) and (k) of 10 CFR 50.49 and sections
5.2.2
and
5.2.6 of the
DOR Guidelines,
FP&L could not show that field-run cables to
two Limitorque motorized valve actuators
(HOVs)
(Tag Nos.
HOV-3-880B and
hlOV-4-880B) were qualified in that they were not traceable
to qualified
cable
because
they had incorrect cable tag numbers
and
no other identify-
ing markings in the
HOV.

2918) were not installed in a condition similar to that in which they were
tested
in that the splices for SOY-4-2914 and
2916 had insufficient over-
lap over wire insulation
and were installed over the
SOV pigtails'raided
jacket.
One splice for SOY-4-2918 was bent beyond minimum tested
bend
radius
and was also installed over
a braided pigtail jacket.
