REPORTABLE. OCCURRENCE '250-80-.04, UPDATE 1

'LICENSEE EVENT REPORT PAGE TWO Additiona1 Cause Descri tion and Corrective Actions:

The,fins in the'rea of'he tube leak were displa'ced and the tube was slightly indented.

This probably resulted from a foreign object being forced into the radiator by the cooling fans.

This damage-coupled with degradation induced by the 'external environment was the probably root cause of the leak.

The leak was repaired with an epoxy patch.

New radiator cores were ordered.

The integrity of both the.A 6 B EDG radiators was verified by visual examination.

The new rad ator cores were installed in September 1980.
