The root cause of the oroblem was

The biomass valve on the 3B feedwater flow transmitter was not fullv closed w

h an erroneous flow signal to the plant computer.

Additionally, the NIS was found to be improperly adjusted..

Corrective action was taken to close the byoass and to properly adjust the NIS.

LICENSEE EVENT REPORT PAGE TWO Additional Cause Description and Corrective Actions:

The bypass valve on the 3B feedwater flow transmitter was not fully closed resulting in an erroneous flow signal to the plant computer.

The valve was probably unseated accidentally when the transmitter was retubed during the refueling outage.

Additionally, miscalibration (low) of the RES and subsequent misad)ustment of the coarse gain on th'e power range drawers resulted in masking radial offsets which would have been detected by the plant compute'.

Corrective action will include. a periodic check of plant computer channels for calorimetric data with other plant instruments.

Additionally, the procedure detailing the startup sequence following refueling will be revised to require a manual calorimetric prior to reaching threshold power (93K).
