LICiBlSEE EVENT REPOBT PAGE Dv'0 Additional Cause Descri on and Corrective Actions:

1 The failure of the."A" AHJ pump to start resulted fran inoperability of CV-3705 ("A" AEN steam turbine pressure control valve). We root cause of this inoperability was a misalignment between the manifold plugs of the pneumat.c controU.er manifold assenbly and its naunting base assembly due to loose mounting screws. The misalignment was a randan event.

Existing preventive maintenance and AHV system periodic testing will provide assurance that a reoccurrence of this type of event will be promptly detected.
