During power operation, the B CVCS holdup tank was ruptured as a result of low internal tank pressure.

The most probable cause of the low internal pressure was condensation of steam inside the tank when a large quantity of relatively cool water from a normal letdown contacted the hot contents of the tank.

The tank temperature was abnormally high due to a previous transfer of hot water from the Unit No.

3 Reactor Coolant Dra'in Tank to the B CVCS holdup tank.

During this occurrence, the cover gas system could not respond rapidly enough to replace the condensed steam.

The tank rupture resulted in a release of a gaseous mixture with a total activ-ity of 5.6 curies; this represents 0.0059% of the quarterly gas release limit.
