SUBJECT'RC INSPECTION REPORT 50-315/316-97002(DRP),
NOTICE OF
VIOLATION,AND NOTICE OF DEVIATION
Dear Mr. Fitzpatrick:
On February 15, 1997, the NRC completed an inspection at your D. C. Cook
1
and 2 reactor facilities. The enclosed report presents the results of that inspection.
During the 6-week period covered by this inspection report, the inspectors observed that
personnel operated the plant appropriately, generally followed procedures,
and properly
performed engineering evaluations.
Based on the results of this inspection, seven violations of NRC requirements
and one
deviation from a commitment were identified. The violations are cited in the enclosed
Notice of Violation (Notice) and the circumstances
surrounding them are described in detail
in the subject inspection report.
The violations are of concern because they involve
procedures that cannot be followed as written and prccedure adherence
problems
identified by an NRC inspector during the receipt of new fuel at D.C. Cook. The Notice of
Deviation addresses
your failure to meet a commitment during movement of heavy loads.
These concerns are heightened by the number of procedural adherence
and inadequate
procedure problems observed over the last year resulting in the issuance of violations.
For
example, Inspection Report 95014 documented
an improperly set Centrifugal Charging
Pump overcurrent relay due to a personnel error caused in part by a poorly written
procedure. Inspection Report 96011 documented
a failure to follow a temporary
modification procedure when altering a HELB boundary.
Additional procedure adherence
and inadequate
procedure problems were documented
in Inspection Reports 96011 (IRC
test procedure inappropriate to circumstances), 96014 (unit supervisor failed to follow
procedure), and 96015 (failure to reinstall control power fuses, and failure to control fuses
in accordance with plant procedures).
Further, one of the seven violations documented
in
this report concerned foreign rriaterial entering the new fuel vault. This violation is of
particular concern because
Inspection Report 96002 documented
other examples of
foreign material entering the new fuel vault.

and inadequate
procedure problems at D. C. Cook. These problems are underscored
by the issues
identified by the NRC during observation of during new fuel receipt inspections performed
by maintenance
personnel.
Three procedures were used during new fuel receipt and were
required to be "in hand" as indicated by a double asterisk on each procedure.
However,
the NRC identified that the procedures not were not in the vicinity of maintenance
personnel performing the new fuel receipt activities. After retrieval of the procedures
by
maintenance
personnel, it'was identified that procedures
could not be followed as written.
Further, the oversight provided by a nuclear engineer and a maintenance supervisor failed
to identify the procedural compliance errors occurring around them.
The complacency
exhibited by licensee staff towards the performance of new fuel receipt inspection
activities raises questions about the ability of personnel to use and follow procedures at
D. C. Cook.

enclosed Notices when preparing your response.
In addition to responding to the specific
violations, your response
should also address any broad corrective actions you have taken
or willtake in addressing
procedural adherence
and procedural adequacy issues at D. C.
