CPPR-143 NOTICE Of VIOLATION As a result of the inspection conducted on October 7-10,
- 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Appendix B of 10 CFR 50, Criterion V, as implemented by paragraph
- 5. 1 of the guality Assurance Program documented in Appendix A to the PSAR requires that "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions procedures, or drawings...."
Bechtel specification no.
13-CH-365,. an. instruction entitled "Installation specification for the forming, placing, finishing and curing of concrete,"
- states, in part, in Section 10.3 that, "Debris, mud, snow and ice shall-be removed from spaces to receive concrete and the reinforcement and other metal to be embedded shall be cleared of all coating which might impair the bond...."
Contrary to the above, on October 7, 1980, debirs, including plastic foam, wood, paper, glass, tape and similar trash, was entrapped in concrete resulting in three pockets of unbonaed=concrete extending benind;-the --

Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
