This letter refers to the Inspection conducted by Messrs.
J.
H. Eckhardt and G. Hernandez on October 7-10,
- 1980, as documented in your letter of December 24, 1980, of activities authorized by the Nuclear Regulatory Commission (NRC) Construction Permit Nos.
CPPR-141, 142 and 143.
During this Inspection, one (1) item of noncompliance was identified.
Our response to this item of noncompliance (Severity Level 5 Violation (Supplement II)) is presented in the enclosed Attachment A.

ATTACHMENT A Docket No. 50-530 Construction Permit No.
CPPR-143 Notice of Violation As a result of the inspection conducted on October 7-10,
- 1980, and in accordance with the Interim Enforcement Policy,'5 FR 66754 (October 7, 1980),
the following violation was identified:
Appendix B of 10CFR50, Criterion V, as implemented by Para-graph 5.1 of the Quality Assurance Program documented in Appendix A to the PSAR requires that "Activities affecting quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circum-stances and shall be accomplished in accordance with these instructions, procedures or drawings...."
Bechtel Specification No.
13-CM-365, an instruction entitled "Installation Specification for the Forming, Placing, Finish-ing and Curing of Concrete",
- states, in part, in Section 10.3, that "Debris, mud, snow and ice shall be removed from spaces to receive concrete and the reinforcement and other metal to be embedded shall be cleared of all coating which might impair the bond...."
Contrary to the above, on October 7, 1980, debris, including plastic foam, wood, paper,
- glass, tape and similar trash, was entrapped in concrete resulting in three pockets of unbonded concrete extending behind the first layer of reinforcing steel at two construction joints at the 68'levation of the Unit 3 Auxiliary Building, a safety-related structure.
The concrete placements associated with these pockets of entrapped debris were Placement Nos.
- 3A035, 3A036,
- 3A038, 3A044 and 3A119.
This is a Severity Level 5 Violation (Supplement II).
~Res arise The subject violation was in an area of highly congested reinforcing.
To attempt to preclude what happened, openings were provided in the forms to permit cleaning accessibility.
The responsible QCE and AFE inspected the area prior to placement.
The three pockets identified were missed during the final inspection of the concrete pour prior 'to placement.
On October 8,
- 1980, NCR No. C-A-2187 was written to document the noncon-for'mance and was dispositioned to repair in accordance with Specification No.
13-CM-365, Section 16'.0.
Emphasis on cleanliness requirements in all concrete placements, especially highly congested areas is continuously stressed with all responsible per-sonnel.
Full compliance was achieved by November 17, 1980.
