This refers to the routine safety inspection

conducted

by Hr.

C.

Cox of this

office on Harch

28 - April 1,

1994.

The inspection

included

a review of

authorized activities at your D.

C.

Cook Nuclear Plant facility.

At the

conclusion of the inspection,

the findings were discussed

with those

members

of your staff identified in the enclosed

report.

Areas

examined during .the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of a selective

examination of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The violation is of concern

because

there

were two

occasions

for personnel

to note inadequacies

in their procedures.

Had the

personnel

secured

the evolutions until the inadequacies

could

be resolved,

personnel

contaminations

may have

been

avoided.

The liquid radioactive

waste

system requires'valve

manipulations

by auxiliary operators

and radioactive

waste handlers,to

be closely coordinated

to operate successfully.

Because

two

separate

groups

are involved, close coordination,

good communications,

and

accurate

procedures

are required.

None of this was evident in the events that

led to the resin spill.

You are required to respond

to this letter

and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should

document

the specific actions

taken

and

any additional

actions

you plan to prevent recurrence.

Such actions

should include

a review

of similar evolutions that require coordination

between different groups

and

the steps

taken to improve the coordination

and communications

between

such

groups.

After reviewing your response

to this Notice, including your proposed

corrective actions

and the results of future inspections,

the

NRC will

determine

whether further

NRC enforcement

action is necessary

to ensure

compliance with NRC regulatory requirements.
