FAILURE OF WESTINGHOUSE STEAM GENERATOR TUBE MECHANICAL PLUGS (TAC NOS.

73164 AND 73165)

On May 15,

1989, NRC Bulletin 89-01:

Failure of Westinghouse Steam Generator Tube Mechanical Plugs was issued to all holders of operating licenses or construction permits for pressurized water reactors (PWRs).

The bulletin identified concerns with certain mechanical plugs supplied by Westinghouse and requested that licensees determine whether these plugs were installed in their steam generators and, if so, that an action plan be implemented to ensure that these plugs will continue to provide adequate assurance of reactor coolant system (RCS) pressure boundary integrity under normal, transient, and postulated accident conditions.

By letter dated June 20, 1989, (AEP:NRC: 1096) you responded to the subject bulletin for D.

C.

Cook Nuclear Plant Units 1

and 2.

You indicated that Unit 2 is operating with new steam generators and, therefore, has no mechanical plugs installed.

You further indicated that 538 plugs of the suspected heats were installed in the Unit 1 steam generators and indicated that they would be repaired or replaced during the 1990 Unit 1 refueling outage.

The staff considers this item closed for Unit 2.

The staff notes that although Amendment No.

151 was issued to Facility Operating License No. DPR-58 on December 5, 1990, to allow sleeving of defective generator

tubes, no sleeving took place during that refueling outage.

Please confirm within 30 days receipt of this letter that, as stated in your June 20, 1989 letter, the 538 mechanical plugs of concern in Unit 1 have been either repaired or replaced during the 1990 refueling outage.

Upon receipt of your confirmation, we will consider this item closed for D. C.

Cook Unit 1.
