K. Heller of this office on April 8-16,
1987 of activities at the
Donald C.
Cook Nuclear Plant, Units
1 and 2, authorized
by
NRC Operating
Licenses
No.
and to the discussion
of our findings
with Mr. W. G. Smith, Jr.,
and others of your staff during and at the
conclusion of the inspection.
The inspection
focused
on the circumstances
surrounding Unit
1 operation
with Reactor Coolant System temperature
and pressure
outside the limits
of applicable Technical Specifications.
The enclosed
copy of our inspection
report identifies specific areas
examined during the inspection.
Within these
areas,
the inspection consisted of a selective
examination of procedures
and
representative
.records,
observations,
and interviews with personnel.
Based
on our review of the circumstances
surrounding this event it appears
that breakdowns
in the areas
of sh'ift oversight,
communications,
and execution
of on-shift responsibilities
permitted
a potentially significant violation of
technical specifications
which if not corrected,
could ultimately have
challenged reactor coolant
system integrity.
Accordingly, we determined that
an Enforcement
Conference
was necessary
to discuss
the root causes
of this
event
and your corrective actions to prevent recurrence,
This conference,
which was held
on April 27,
1987,
focused
on the breakdowns
mentioned
above.
You will be notified by separate
correspondence
of the results of this
conference.
We are releasing this report at this time for your information.
You will
be notified by separate
correspondence
of our decision regarding
enforcement
action based
on the findings of this inspection
and the Enforcement
Conference.

is required until you are notified of any proposed
enforcement action.
