I AppendixA NOTICE OF VIOLATION American Electric Power Service Corporation Docket No. 50-315 Docket No. 50-316 As a result of the investigation conducted during the period July 30,
- 1980, thru April 8, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
10 CFR Part 50, Appendix B, Criterion V, states in part, "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances...instructions, procedures, drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Paragraphs 1.0 and 7.5.1-4 of Donald C.
Cook Nuclear Power Plant Procedure No. 12-QHP-2270.QC.001, "QC Inspection on the Installation and Repair of Fire Barrier Penetration Seals," state in part:
"1.0 Purpose...to establish the Quality Control requirements for the installation and repair of fire penetration seals."
"7.5.1-4 Identify the second sample with the material lot number..."
Contrary to the above requirements, Quality Control Inspectors were not documenting the lot number of the fire seal material.

Paragraphs 7.4 and 7.6 of the above procedure state:
"7.4 Perform dispense ratio test (as shown below) once per'hift and sign off Step 7.2.3-2 of the "Sign Off Sheet" if result is acceptable."

Contrary to the above, Quality Control Inspectors were preparing the "Sign Off Sheet" on a bi-weekly to monthly basis, instead of preparing the form at the completion of their inspection of the finished seal.
