NOTICE OF VIOLATION American Electric Power Company Docket No. 50-315 Docket No. 50-316 Based on the results of the NRC inspection conducted on May 21-24,

1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as noted below.

This item is an infraction.

Technical Specification 6.8.1 stated in part that "Written procedures for the repair or replacement of equipment should by prepared prior to beginning work."

IWC 4200 of ASME Section XI, 1974 Edition, references the rules of IWB 4000 for making repairs.

IWB 4140 requires a copy of the repair program be made available to the enforcement and regulatory authorities having jurisdiction at the plant site.

IWA 4240 states

"(a) When components, either prior to operation or during the service lifetime require modifications to saftisfy the requirements of this Division, the repair and inspection procedures of this Division shall be used to affect the modification.

(b) In the event that components in service are to be replaced, or components are to be added to a system, the welds attaching the components to the system shall be constructed in accordance with these repair and inspection procedures."

IWB 4423 states in part, "Each repair operation shall be performed in accordance with a procedure delinating all of the requirements of the complete repair cycle IWB 3125, (b) states in part, "Evaluation analysis of examination results as required by IWB 3123 shall be submitted to the regulatory'uthority having jurisdiction at the plant site "

Contrary to the above, on May 21, 1979, the inspector observed repair of the feedwater nozzle/piping had been started without an established repair procedure.

A procedure for the repair activities was developed and issued on May 28, 1979.
