SUBJECT: NRC INSPECTION REPORT 50-528/98-09; 50-529/98-09; 50-530/98-09 AND
NOTICE 'OF VIOLATION
Dear Mr. Levine:
From November 15 through December 26, 1998, an inspection was conducted at your Palo
Verde Nuclear Generating Station, Units 1, 2, and 3, reactor facility. The enclosed report
presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facilitywas generally characterized by safety-conscious
operations, sound engineering
and maintenance practices, and careful radiological controls; however, violations of NRC
requirements were identified.
The first violation involved the failure of an operator to correctly position a valve in the shutdown
cooling system, as required by procedure.
This is a repeat violation. The second violation
involved the failure to comply with the Maintenance Rule in that the postaccident sampling
system experienced frequent component failures and inadequate actions were taken to correct
the failures.
The NRC has concluded that information regarding the reasons for these violations and the
corrective actions taken or planned to correct the violation and prevent recurrence are already
adequately addressed
on the docket in the attached inspection report. Therefore, you are not
required to respond to this letter unless the description herein does not accurately reflect your
corrective actions or your position.
In that case, or if you choose to provide additional
information, you should followthe instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
