SUBJECT: NRC INSPECTION REPORT 50-528/9M%; 50-529/98%6; 50-530/98-06 AND
Dear Mr. Levine:
From July12 through August 22, 1998, an inspection was conducted at your Palo Verde
Nuclear Generating Station, Units 1, 2, and 3, reactor facilities. The enclosed report presents
the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facilitywas generally characterized by safety-conscious operations, sound engineering
and maintenance'practices,
and careful radiological controls; however, a.violation of NRC
requirements was identified.
The violation occurred when operations personnel became narrowly focused on one
requirement and failed to recognize that their actions would result in entry into Technical
Specification 3,.0.3. "
.
The,NRC has concluded that information regarding'the reasons for the violations, the corrective
actions taken and planned to correct the violation and prevent recurrence are already
adequately addressed
on the docket in Licensee Event Report 50-530/98-001 for the violation.
Therefore, you are not required to respond to this letter unless the description therein does not
accurately reflect your corrective actions or your position. In that case, or ifyou choose to
provide additional information, you should followthe instructions specified in the enclosed
Notice.
