SUBJECT: NRC INSPECTION REPORT 50-528/98-11; 50-529/98-11; 50-530/98-11 AND
Dear Mr. Levine: .
From January 27 through March 5, 1998, a special inspection was conducted at your Palo Verde
Nuclear Generating Station, Units 1, 2, and 3 reactor facilities in response to a sulfuric acid spill
in the north yard of Unit 2 that involved radioactive contaminants.
The on-site portion of the
inspection was performed from January 27 through 30, 1998. The enclosed report presents the
scope and results of that inspection.
A violation of NRC requirements was identified for the failure to make an entry in the control
room log describing the abnormal condition observed in the Unit 2 sulfuric acid storage system
on January 22, 1998. This violation is of concern because
it represents
one of several
communication barriers that failed leading up to the sulfuric acid spill on January 23, 1998.
Further, actions taken in response to previously identified logkeeping weaknesses,
below
regulatory thresholds, did not prevent this failure. The violation is cited in the enclosed Notice of
Violation (Notice) and the circumstances
surrounding this violation are described in detail in the
enclosed report.
Please note that the NRC has concluded that information regarding the reason for the violation,
the corrective actions taken and planned to correct the violation and prevent recurrence are
already adequately addressed
in your Significant Root Cause Investigation Report 2-8-033
dated February 11, 1998, and summarized in Attachment 4 to the enclosed inspection report.
Therefore, you are not required to respond to this letter unless the description therein does not
accurately reflect your corrective actions or your position.
In that case, or ifyou choose to
provide additional information, you should followthe instructions specified in the enclosed
Notice.
