E CLOSU NOTICE OF VIOLATION(Revised)
Arizona Public Service Company Palo Verde Nuclear Generating Station Docket No.: 50-529 License No.: NPF-51 During an NRC inspection conducted on September 7 through October 18, 1997, one violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
Technical Specification 3.4.1.4.1 requires that, with the reactor in Mode 5 and the reactor coolant loops filled, at least one loop of shutdown cooling must be in operation.
Paragraph 4.4.1.4.1.2 requires that, for the shutdown cooling loop to be in operation, it must be circulating reactor coolant at a flow rate greater than or equal to 3,780 gpm.
Contrary to the above, from approximately 8 p.m. on October 5, 1997 until about 11:55 a.m. on October 6, 1997, Unit 2 was operated in Mode 5 with the reactor coolant loops filled, with approximately 3380 gpm of shutdown cooling flow, which is less than the required 3,780 gpm. Immediately upon the condition being recognized by the operating crew, corrective actions to re-establish the required shutdown cooling flowwere initiated.
This is a Severity Level IVviolation (Supplement
- 1) (50-529/97016-01).
The NRC has concluded that the information regarding the reason for the violation, the corrective actions taken, the actions planned to correct the violation and prevent recurrence, and the date when full compliance was achieved are already adequately addressed on the docket in your letter dated November 26, 1997.
However, you are required to submit a written statement or explanation pursuant to 10 CFR Part 2.201 ifthe description therein does not accurately reflect your corrective actions or your position considering this revision to the Notice of Violation.
In that case, or ifyou choose to respond, clearly mark your responseas a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector at the facilitythat is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
Because your response, ifany, will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.
However, ifyou find it necessary to include such information, you should clearly indicate the specific information that you desire not to be placed in the PDR, and provide the legal basis to support your request for withholding the information from the public.
