SUBJECT:
NRC INSPECTION REPORT 50-528/97-19; 50-529/97-19; 50-530/97-19 AND
Dear Mr. Levine:
An NRC engineering team inspection was conducted
May 19 through June 19, 1997, at
your Palo Verde Nuclear Generating Station, Units 1, 2, and 3, reactor facilities. The team
reviewed engineering activities to evaluate the effectiveness of the engineering
organization
in performing routine and reactive site activities, including the identification
and resolution of technical issues and problems.
The enclosed report presents the scope
and results of that inspection.
The team found good engineering performance during the review of engineering work
products, such as design modifications, deficiency work orders, desIgn calculations,
operability evaluations
and disposition of condition reports/disposition
requests.
However,
the team also identified a violation involving inadequate
corrective actions taken for
containment spray water hammer events that were recently experienced
in Units 2 and 3.
in addition, the team identified a violation involving the failure to report eix unit 3 main
steam safety valves that were found to be set outside your technical specification
tolerances.
These violations are cited in the enclosed
Notice of Violation (Notice) and the
circumstances
surrounding the violations are described
in detail in the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed
Notice when preparing your response.
The NRC will
use your response,
in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
One unresolved item was identified-concerning the lack of a 10.CFR 50.59 safety
evaluation for the use of.a computer. code. for.determining the, source, term,ofpour fuel ..
handling accident analysis in a manrter differe'nt than specified "in your Updated Safety
Analysis Report: This matter is discussed
in Section E1.6 of the enclosed inspection
report. -As discussed
at the exit m'ecting, your staff was still researching
previous safety
evaluations for the noted condition.
