ENCLOSURE 1 NOTICE OF VIOLATION Arizona Public Service Company Palo Verde Nuclear Generating Station Docket Nos.:
50-528 50-529 50-530 License Nos.: 'PF-41 NPF-51 NPF-74 During an NRC inspection conducted on May 4 through June 14, 1997, one violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
Technical Specification 6.8.1 requires, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Regulatory Guide 1.33, Revision 2, Appendix A, Section 1.c., requires written procedures for equipment control.
In addition, Section 1.k. requires written procedures for maintenance of minimum shift complement and call-in of personnel.

Procedure 40AC-OZZ06, "Locked Valve, Breaker, and Component Control,"
Revision 11, describes the method established to require administrative controls for locking valves, breakers, and components required to be locked.
Step 3.1.5, states, for valves, the labeling tag shall be attached with wire cable and a sealing device shall be attached to the cable to prevent its manual operation.
Contrary to the above, on May 2, 1997, the inspectors identified two Unit 3 and,one Unit 1 locked closed valves that had the tag attached to the wire cable and a sealing device attached to the wire cable that was loosely wrapped around the valve, allowing manual operation of the valve, 2.
Procedure 40DP-9OP19, "Locked Valve, Breaker, and Component Tracking,"
Revision 22, implements the documentation method required by Procedure 40AC-OZZ06, "Locked Valve, Breaker, and Component Control."
Step 2.2, states, in part, that personnel operating locked valves are responsible for completing Appendix C.for the valve they are operating.
Contrary to the above, on June 2, 1997, an auxiliary operator respositioned the Unit 3 Auxiliary Feedwater Pump N discharge Valve AFNV013, as required by Equipment Clearance Order 9700609, and did not complete Appendix C of the procedure.
Valve AFNV013 was controlled by Procedure 40AC-OZZ06.

Procedure 40DP-9OP02, "Conduct of Shift Operations,"
Revision 2, establishes requirements for the duties and activities performed by the on-shift operating personnel.
Step 3.1.1, states that the minimum shift crew composition for auxiliary operators in Modes 1-4 is four.
Contrary to the above, on October 26, 27, and 28, 1996, while Unit 2 was in Mode 1, the operations crew composition consisted of three auxiliary operators which is below the required minimum number.
This is a Severity Level IV violation (Supplement
- 1) applicable to Units 1, 2, and 3 (50-528;529;530/97006-01).
The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence and the date of full compliance was achieved is already adequately addressed on the docket in this inspection report.
However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description herein does not accurately reflect your corrective actions or your position.
In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U. S. Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within the 30 days of the date of the letter transmitting this Notice of Violation (Notice).
