NRC INSPECTION REPORT 50-528;-529;530/97-09
AND NOTICE OF
VIOLATION
Dear Mr. Levine:
A special NRC inspection was conducted
March 11-21, 1997, at your Palo Verde Nuclear
Generating Station, Units 1, 2, and 3, reactor facilities, with in-office inspection and
followup conducted through April 4, 1997.
This inspection followed up on the notice of
unusual event of March 6, 1997, when there was a partial drain down of the Unit 3 spent
fuel pool ~ An onsite exit meeting was held on March 21, 1997.
Some information, which
had been requested
by the inspectors, was not received during the onsite portion of the
inspection.
Following receipt and review of that information, a supplemental
telephonic
exit meeting was conducted
on April 4, 1997, with Ms. A. K. Krainik and members of your
staff. The enclosed report presents the scope and results of the inspection.
The inspection identified three violations.
The first violation, (involving four examples)
is of
concern because
it indicates undocumented
and poorly coordinated work performance,
a
lack of procedural specificity, insufficient attention to detail, and a failure to properly
implement preventive maintenance.
The second violation is of concern because it indicates
a long- tanding problem relative to the design control of spent fuel pool gate seals.
The
third violation is of concern because
it indicates inadequate
procedural instructions for the
maintenance
of safety-related
equipment.
In the aggregate,
these violations are indicative
of a lack of sufficient oversight of processes
and activities that have the potential to affect
spent fuel pool water level ~
These violations are cited in the enclosed
Notice of Violation (Notice) and the
circumstances
surrounding the violations are described
in detail in the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed
Notice when preparing your response.
The NRC will
use your response,
in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
