NRC INSPECTION REPORT 50-528/96-17; 50-529/96-17; 50-530/96-17 AND
Dear Mr. Levine:
An NRC inspection was conducted
November 17 through December 28, 1996, at your
Palo Verde Nuclear Generating Station, Units 1, 2, and 3 reactor facilities. The enclosed
report presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Palo
Verde facility was generally characterized
by safety conscious operations,
sound
engineering
and maintenance
practices, and careful radiological controls.
However, three
violations of NRC requirements
occurred.
One violation involved three examples of failure
to follow procedures.
One example involved the failure of operations to manually input an
alarm into the safety equipment status system when one emergency'core
cooling system
train was made inoperable for testing.
The two additional examples involved failure to
follow procedures
during the installation of a temporary restraining device on the shaft of a
reactor coolant pump during the Unit 1 refueling outage.
Although the installation of the
restraining device was supervised
by the mechanical maintenance
engineering organization,
It was not described by work instructions and was not adequately covered under an
equipment clearance.
The second violation included two examples of failure to follow written instructions.
In
one example, maintenance
and operations failed to adequately discuss
and implement the
requirements
of a charging pump seal leakoff drain system test.
As a result, a significant
amount of water entered the crankcase
oil of a charging pump, placing it in a degraded
condition.
In the second example, workers failed to implement the requirements
of a
piping design specification to verify pipe support to piping clearance.
The third violation
involved the failure of an escort to comply with requirements to maintain control of his
visitor inside a plant vital security area.
We are concerned that these situations regarding
failure to follow procedures
represents
a recent decline in your performance
in this area
and urge your continued attention to improve performance.
During this inspection period there were three problem situations which were not
documented
in a timely manner using your corrective action program method.
Over the
past year, the staff at Palo Verde has earned
a reputation for being diligent at identifying
problems and performing thorough evaluations
of. significant issues.

that the problem identification and resolution issues
in this report appear to indicate a
degrading trend and urge your continued attention to improve performance
in this
important area.
These violations are cited in the enclosed
Notice of Violation (Notice) and the
circumstances
surrounding the violations are described
in detail in the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed
Notice when preparing your response.
The NRC will
use your response,
in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter,
its enclosure(s),
and your response
will be placed in the NRC Public Document Room
(PDR). To the extent possible, your response
should not include any personal privacy,
proprietary, or safeguards
