Dear Mr. Stewart:

e

An NRC inspection

was conducted

June

2 through July 13,

1996,

at your Palo

Verde Nuclear Generating Station,

Units 1,

2,

and

3 reactor facilities.

The

enclosed

report presents

the

scope

and results of that inspection.

During the 6-week period covered

by this inspection,

your conduct of

activities at the Palo Verde facilities was generally characterized

by safety

conscious

operations,

sound engineering

and maintenance

practices,

and careful

radiological controls.

During this inspection,

the

NRC determined that two violations of NRC

requirements

occurred.

The firstviolation involved the. failure of

Instrumentation

and Controls maintenance

engineering to recognize that

a

change to the Unit 2 gualified Safety Parameter

Display System represented

a

change to the facility as described

in the safety analysis report

and to

review the change to determine

whether it represented

an unreviewed safety

question.

The second violation involved the failure of an individual to

process

themselves

and their hand carried

items through security search

equipment prior to entering the plant protected

area.

This item is of concern

since it represents

a

common practice for janitorial staff performing work at

the plant protected

area

access,

and this practice

was accepted

by plant

security.

These violations are cited in the enclosed

Notice of Violation (Notice)

and

the circumstances

surrounding

the violations are described

in detail

in the

enclosed

report.

Please

note that you are required to respond to this letter

and should follow the instructions specified in the enclosed

Notice when

preparing your response.

The

NRC will use your response,

in part, to

determine

whether further enforcement

action is necessary

to ensure

compliance

with regulatory requirements.

In accordance
