REPORT 50-528/95-14;

50-529/95-14;

50-530/95-14

AND

NOTICE OF VIOLATION

This refers to the inspection

conducted

by Mr. K.

E. Johnston

and others of

this office on July

2 through August

13,

1995.

The inspection

included

a

review of activities authorized for your Palo Verde Nuclear Generating

Station,

Units 1,

2,

and 3, facility.

At the conclusion of the inspection,

the findings were discussed

with you

and

members of your staff,

as identified

in the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license

were. conducted safely

and in

accordance

with NRC requirements.

Based

on the results of this. inspection,

certain licensed activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The violation involved failure to follow procedures

for

the use of rigging equipment.

The rigging equipment,

which was

used to lift a

High Integrity Container

loaded with radioactive

spent resin,

was undersized

and failed during the lift, allowing the container to drop approximately

seven

feet onto the ground.

Although the container

was not damaged

and radioactive

material

was not released,

this violation is of concern

because

of the

.

potential for significant radioactivity release,

and

because

of your failure

to correct similar concerns

that

had

been previously identified by your

Nuclear Assurance

organization.

In addition,

the report documents

a number of operations

performance

issues

which appear contrary to the

improvement noted in operations

performance

since

the

end of 1994.

In one instance,

operator

performance

weaknesses

contributed

to

a reactor trip.

In two other instances,

operator

performance

weaknesses

resulted

in failure to recognize that two trains of auxiliary feedwater

equipment

had

been simultaneously

removed

from service for maintenance,

and

operators

failed to properly evaluate

and resolve

an observed

estimated

critical rod position discrepancy prior to continuing with a reactor startup.

However, if you find it

necessary

such information, you should clearly indicate the

specific information that you desire

not to be placed in the

PDR,

and provide

the legal basis to support your request for withholding the information from

the public.

directed

and the enclosed

Notice are not subject

to the clearance

procedures

of the Office of Management

and Budget

as required
