AND

NOTICE OF DEVIATION

This refers to the inspection

conducted

by K. Johnston

and others of this

office on February

26 through April 8,

1995.

The inspection

included

a review

of activities authorized for your Palo Verde Nuclear Generating Station,

Units 1,

2,

and 3, facility.

At the conclusion of the inspection,

the

findings were discussed

with you and those

members of your staff identified in

the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within-

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation of

t

activities in progress.

The purpose of the inspection

was to determine

whether activities authorized

by the license

were conducted

safely

and in

accordance

with'RC requirements.

Overall, plant management

addressed

plant problems in an aggressive

and

thorough

manner with minimal

NRC involvement.

An example of this was the

engineering

response

to the fouling of the Unit 2 emergency diesel

generator

coolers.

Maintenance

management

was not as prompt in addressing

the

implication of control element drive mechanism

breaker failures.

It appeared

that the focus

was

on breaker

replacement

and not component operability.

Based

on the results of this inspection,

certain licensed activities appeared

to deviate

from your commitments in the Updated Final Safety Analysis

Report

(UFSAR),

as specified in the enclosed

Notice of Deviation (Notice).

The Notice involved the failure to follow procedures

for the testing of the

standby

gas turbine generators

as specified in the guality Assurance

Plan

described

in Chapter

17 of the

UFSAR.

As a result of not following testing

procedures,

one of the gas turbine generators

picked

up excessive

load when it

was paralleled with offsite power, resulting in a trip of the gas turbine.

The deviation is of concern

since the inspector

observed

poor procedural

compliance

and poor command

and control associated

with the operation of

equipment

designed

to mitigate station blackout conditions.

, This matter is

discussed

in Section

5. 1 of the enclosed report.

One unresolved

item was identified concerning

the conditions which led to the

t

licensee's

request for a Notice of Enforcement Discretion regarding

containment

penetration

overcurrent protection devices.

This matter is

discussed

in Section

2.2 of the enclosed

inspection report.
