AND
This refers to the inspection
conducted
by A. HacDougall
and J.
Keeton of this
office on Narch 6-10,
1995.
The inspection
included
a review of activities
authorized for your Palo Verde Nuclear Generating
Station,
Units
1,
2,
and 3,
facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
The
special
inspection
reviewed the circumtances
of the identification of startup
strainers
in the containment
spray
system in Unit 2 on February
17
and
assessed
the adequacy of the Arizona Public Service
Company's
response
to the
event.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and
observation of activities in progress.
The purpose of the inspection
was to
determine
whether activities authorized
by the license
were conducted
safely
and in accordance
with
NRC requirements.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified
in the enclosed
Notice of
Violation (Notice).
The violation is of concern
because it involves the
failure to take adequate
corrective actions
in three separate
opportunities to
identify that startup strainers
were still installed in the plant.
Although
the initial error that left the strainers
installed in the Unit 2 containment
spray
system occurred
almost
10 years
ago,
the last
two opportunities
to
identify the strainers
occurred
less
than
2 years
ago.
These
two reviews
appeared
to be primarily focused
on justifying why a detailed evaluation
was
not required to prove that all the startup strainers
had
been
removed.
This
is of concern
because
there
have
been other evaluations
where individuals
lacked
a questioning attitude which contributed to weak corrective actions
(most notably the failure to identify the bent connecting
rod in an emergency
diesel
generator).
We do recognize that the recent significant actions
you
have taken to upgrade your corrective actions
program
have resulted
in an
overall
improvement in the your resolution of safety issues.
We found your investigation of the root cause of this event to be thorough
and
to demonstrate
a high level of management
involvement.
We continue to have
confidence that your personnel
can conduct quality investigations of safety

However, if you find it
necessary
to include such information, you should clearly indicate the
specific information that you desire
not to be placed
in the
PDR,
and provide
the legal basis to support your request for withholding the information from
the public.

directed

and the enclosed
Notice are not subject
to the clearance
procedures
of the OfFice of Management
