This refers to the inspection
conducted
by Hr. K. Johnston
and others of this
office on October
16 through
November 30,
1994.
The inspection
included
a
review of activities authorized for your Palo Verde Nuclear Generating
Station,
Units 1,
2,
and 3, facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the
,'enclosed
report.
Areas
examined during the inspection
are identified'n the report.
Within
these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted safely
and in
accordance
with NRC requirements.
During this inspection period,
management
made significant changes
in the area
of operations'xpectations,
crew staffing and operations'rganization.
These actions
demonstrated
an understanding
of conduct of operation
weaknesses.
However,
the
NRC identified misaligned valves during the Unit 3
draindown to midloop conditions.
This event
was the subject of a special
inspection
which concluded
on December
21,
1994.
This event is addressed
in a
separate
inspection report
and is not addressed
in this report.
Based
on the results of this inspection,
certain of your activities appeared
to deviate
from your commitments
made to support
a Technical Specification
change
regarding surveillance
requirements
for the Unit 2 125-Vdc vital
batteries,
as specified in the enclosed
Notice of Deviation (Notice).
These
commitments
involved the review of maintenance
activities
on the vital 125-Vdc
equipment
and the control of access
to the plant switchyard.
In two
instances,
work performed which impacted
125-Vdc equipment
was not reviewed
and,
in one instance,
a gate to the plant switchyard
was found open
and
uncontrolled.
These
examples
indicated that management
had not adequately
communicated
the sensitivity of these activities to plant personnel.
While the root cause
was not found for the switching of lamp covers
on the
Unit
2 remote
shutdown panel, identified by a resident
inspector,
the event
highlights the
need for licensee
personnel
to be attentive to discrepant
conditions.

Three unresolved
items were identified concerning the failure of a safety
injection valve to open during testing.
The second
item is discussed
in
Section
2. 1 of the enclosed
inspection report.
Another item, discussed
in
Section
4. 1, involved previously identified weaknesses
in a maintenance
instruction for safety-related
solenoid-operated
valves which were not
corrected prior to work performed
on Unit
1 pressurizer
head vent valves.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and any additional-
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous docketed
correspondence, if the correspondence
adequately
addresses
the required response.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter, its enclosure(s),
and your response will be placed in the
NRC
Public Document
Room (PDR).
To the extent possible,
your response
should not
include
any personal
privacy, proprietary,
or safeguards
information so that
it can
be placed in the
PDR without redaction.
However, if you find it
necessary
to include such information, you should clearly indicate the
specific information that you desire not to be placed in the
PDR,
and provide
the legal basis to support your request for withholding the information from
the public.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork Reduction Act of 1980,
Pub.
