This refers to the inspection
conducted
by Messrs.
T.
O. HcKernon,
T.
R.
Meadows,
and J. I. Tapia of this office and other personnel
on
October
11 through
November 9,
1994.
The inspection
included
a review of
activities authorized for your Palo Verde Nuclear Generating Station,
Units 1,
2,
and 3, facility.
At the conclusion of the inspection,
the findings were
discussed
with Hr. J.
Levine and other members of your staff identified in the
enclosed
report.
Areas
examined during the inspection
are identified in the report.
The
inspection
consisted
of a review of the qualifications of applicants for
operator licenses
at the Palo Verde Nuclear Generating
Stat'ion,
which included
an eligibility determination
and administration of comprehensive
written
examinations
and operating tests.
The results of this inspection
are
documented
on page
2, in the enclosed
report.
Ouring the inspection,
three of the six initial license applicant
crews
examined did not initiate emergency
boration
when presented
with a simulated
anticipated transient without scram.
This behavior is of special
concern to
the
NRC because it reflects failure to perform properly required
emergency
operating
procedure
immediate actions to control reactivity.
Further,
as
documented
in the report,
informal training on procedure
usage
and ope'rating
philosophy contributed to the observed
misperformance.
This concern is
exacerbated
because
similar performance
was observed
on the
1993 initial
license
examinations,
as documented
in Operator Licensing Report 93-03.
While
this misperformance
was not observed
in our 1994 licensed operator
requalification examinations,
the
NRC is concerned
that adherence
to the
operators'mergency
operations
safety function flowcharts
and the importance
of maintaining reactivity control through
emergency boration,
as well as other
methods,
may not
be sufficiently stressed
in your initial licensed
operator
tra'ining program.

why the misperformance
observed
in 1993 license
examinations
was substantially
repeated
again this
year
and what future actions
you anticipate to preclude further recurrence.
Also, please
explain
how your emergency operating
procedures
under
development,
including the implementing training program, will enhance
completion of immediate action steps.
Please
reply within 30 days of receipt
of this letter with the above information, or your projected
schedule
to
provide it.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the
NRC Public Document
Room.
