This refers to the inspection
conducted
by K. Johnston
and others of this
office,
on June
12 through July 23,
1994.
The inspection
included
a review of
activities authorized for your Palo Verde Nuclear Generating Station,
Units 1,
2,
and 3, facility.
At the conclusion of the inspection,
the findings were
discussed
with those
members of your staff identified in the enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted safely
and in
accordance
with NRC requirements.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation concerned
the failure to perform
a channel
calibration check in accordance
with the Technical Specification surveillance
requirements.
In April 1994,
the Plant Review Board questioned
how the
surveillance
requirements
were being met, but failed to follow up to resolve
the matter until the requirements
were again questioned
in June
1994.
During the inspection,
several
findings were identified that highlighted
an
apparent
lack of plant management
in the field.
For example,
a month after
a
refueling outage
the inspector noted material condition weaknesses
in Unit 3
which could
be attributed to outage work. It was also noted that, during this
period, licensee
management
focused
a substantial
amount of time on the
reorganization
selection
process.
With the implementation of the first wave
of organization
changes
completed,
we encourage
you to reassess
the priority
placed
on management field presence
as stated
in your response
to the
1993
Systematic
Assessment
of Licensee
Performance
report.
