This refers to the inspection
conducted
by K. Johnston of this office,
and the
inspectors
identified in the enclosed
report
on March 29 through
Hay 7,
1994.
The inspection
included
a review of activities authorized for your Palo Verde
facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observation
of
activities in progress.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation concerned
the failure of nonlicensed
operators
to correctly rack out
a motor control center breaker.
In addition, certain of your activities appeared
to deviate
from your
commitments
made in
a response
to
a previous Notice of Violation, as specified
in the enclosed
Notice of Deviation (Notice).
In an August 7,
1992, letter in
response
to
a Notice of Violation, the licensee
committed to perform technical
reviews for plant instrumentation
found out of calibration.
The inspector
subsequently
found that these
reviews were not being I:erformed for a
substantial
population of safety-related
instrumentation.
Both the violation and deviation involved issues
previously addressed
by the
inspectors
in which corrective actions
had not been
thorough
and
had not
prevented
recurrence.
These
examples,
and others
in recent inspections,
indicate that, while the immediate actions
taken
by management
in response
to
a problem tend to be aggressive,
the subsequent
followup has not consistently
ensured
that the problem is resolved.
