SUBJECT:

CLOSURE OF CONFIRMATORY ACTION LETTER This letter refers to the Confirmatory Action Letter (CAL) dated October 4,

1993, as revised by letters dated November 8 and 23, 1993.

The CAL details the September 24,

1993, conference call discussions between Arizona Public Service Company (APS) and..the Nuclear Regulatory Commission (NRC) on possible axial indications in the'pper region of the tube bundle in steam generators at Palo Verde.

In the CAL, we confirmed APS'ommitments to: (1) shut down Unit 3 for eddy current testing (ECT) inspection of both steam generators, (2) continue the review of Unit 3 ECT data to identify indications which were not identified in refueling outage 3R3 by bobbin coil ECT and to provide a written summary of the review, (3) continue to implement the Unit 1 Steam Generator Inspection

Plan, (4) complete the commitments made in your letter dated July 30, 1993, concerning changes to Emergency Operating Procedures, operator
training, and leakage monitoring, and (5) conduct operation of Unit 3 to take advantage of some of the preventive measures which can be taken to reduce outer diameter stress corrosion cracking rates.

By letter dated December 3,

1993, APS indicated that it had completed the actions contained in the CAL.

With the completion of the action items, APS has satisfied the conditions of the CAL.

We note"that APS has implemented additional actions to monitor for steam generator

leakage, including the installation of N-16 monitors in all three
units, and will take certain actions in the event of increasing leakage.

These actions, including plant shutdown on leakage of 50 gallons per day, have been incorporated into APS operating procedures.

In addition, APS is operating the units at approximately 85~ power to reduce deposi ts in the upper tube bundle, and has reduced T-Hot to reduce stress corrosion cracking.

Further, APS continues to control steam generator chemistry to mitigate corrosion attack (i.e., molar ratio control, elevated hydrazine levels, increased pH, and periodic power reductions).

Based on the above actions, this CAL is closed.

APS has proposed to further reduce reactor coolant system temperature and has submitted an amendment to the Technical Specifications to implement this fA 9aoai 5ooa2 9aoao~

This would also allow a return to 100$ power operation.

The NRC staff is evaluating this amendment.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice,"

a copy of this letter will be placed in the NRC Public Document Room.
