NOTICE OF VIOLATION Arizona Publ i c Servi ce Company Palo Verde Nuclear Generating Station, Unit 2 Docket No. 50-529 License No. NPF-51 During an NRC inspection conducted during the period April 19 through 30,
- 1993, a violation of NRC requirements was,identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1993),
the violation is listed below:
10 CFR 50.55a (g) (3)(ii) states that components classified as American Society of Hechanical Engineers (ASHE)
Code Class 1,
2 and 3 shall meet the inservice requirements set forth in Section XI of the ASNE Hoi ler and Pressure Vessel Code and Addenda applied to that particular component (including supports)
Arizona Public Service Company, inservi ce inspection program for Palo Verde Unit 2, invokes the requirements of ASME Code,Section XI, 1980 Edition including Addenda through the Minter 1981, para.
IWA-2213(a) which states that the visual examination shall be conducted to determine the general mechanical and structural integrity of components and their supports, such as the presence of loose parts, etc.
The licensee's procedure, "Visual Examination of Support Components,"
73TI-9ZZ18, Revision 4, dated 1/14/91, states that all accessible bolted connections shall be checked manually
( by hand ) for tightness and abnormal conditions shall be identified and evaluated on an examination report.
Contrary to the above, on April 22,
- 1993, Palo Verde Inservice Visual Examination Report 93-2316, for steam generator blowndown piping support SG-048-H017, reported that the component associated with this support was examined and evaluated as acceptable.
On April 27,
- 1993, NRC inspectors performed an independent followup examination of support SG-048-H017 and discovered a nut could be turned by hand on this component, and this abnormal condition was not identified and evaluated in examination report 93-2316 by the licensee.
This is a Severity Level V violation, Supplement I.
Pursuant to the provision of 10 CFR 2=.201, Arizona Public Service Company, is hereby required to submit a written statement or explanation to the U.S.
Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C.
20555, with a copy to the Regional Administrator, Region V, and a copy to the Resident Inspector at Palo Verde Nuclear Generating Station, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation, or if contested, the basis for the disputing the violation, (2) the corrective steps that have been taken and the results
- achieved, (3) the corrective steps that will be taken to avoid further violations, and (4
the date when full compliance will be achieved.
If an adequate reply is not received within the.
time specified in this Notice, the Commission may issue an order or a demand for information as to why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
