EXAMINATIONREPORT 50-528/OL-92-01 From June 1-12, 1992, the NRC administered initial licensing examinations to fourteen Reactor Operator (RO) and three Senior Reactor Operator (SRO) candidates who had applied to operate your Palo Verde Nuclear Generating Station, Units 1, 2, and 3

(PVNGS).

One of the RO candidates failed the written portion of the examination, and was denied a license.

This individual can reapply for a license within 60 days of the date of his license denial notification per 10 CFR 55.35.

All of the other candidates were issued a letter informing them that they had passed the examination.

Their licenses are being held in abeyance pending our receipt of written notification from Arizona Public Service Company (APS) that the emergency operating procedures (EOPs) upon which the candidates were trained and examined have been implemented.

This notification should also affirm that the candidates are proficient in the use these procedures.

Since the previous examination, the examiners noted a substantial improvement in the quality and range of available simulator capabilities.

Also, your training and operating staffs provided good technical support in the validation of the examination test materials.

One concern was identified by the examiners.

It appeared that your alarm response procedures did not provide adequate guidance for operators to consistently mitigate an event involving the slow depressurization of the reactor coolant system (RCS).

The issue is unresolved, and will be further addressed in NRC Inspection Report 50-528/92-22.

This concern and other identified deficiencies regarding your licensed operator training program and plant procedures were discussed with the members of your staff during an exit meeting on June 12,

1992, and are detailed in Enclosure 1, Examination Report 50-528/OL-92-01.

July 23, 1992 In an enclosure to a letter 'received from Mr. J. Levine, Vice President Nuclear Production dated, June 19, 1992, your staff responded to all of the issues discussed at the exit meeting.

This letter, which is included in the examination report, identifies the commitments made by your staff at the exit meeting to resolve the examiners'oncerns.

Your actions will be reviewed during the next visit by a qualified examiner to Palo Verde.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and Enclosure (1) will be placed in the NRC Public Document Room.
