NRC SYSTEM BASED INSTRUMENTATION AND CONTROL (I&C) INSPECTION OF

PALO VERDE UNITS 1, 2,

AND 3

NRC INSPECTION REPORT

NO. 50-528/529/530/92-14

Due to the recent loss of annunciator

event

and subsequent

Augmented

Inspection

Team at Palo Verde Unit 3, the scope,

number of inspectors,

and

time scheduled for this inspection

were reduced to 50X of the planned effort.

The depth

and scope of areas

inspected

were commensurately

reduced.

Areas examined during the inspection

are identified in the enclosed report.

Within these

areas,

the inspection consisted of examination of selected

procedures

and representative

records,

interviews with personnel,

and safety

system walk-downs.

The objective of this inspection

was to verify the functionality of safety

systems

by inspecting

performance related attributes of the system with a

focus

on its associated

instrumentation

and controls systems,

and the ability

of the system to perform its intended design function.

The team also

evaluated your I&C and setpoint programs,

and your commitments to these

programs.

Based

on the results of this inspection,

the team concluded that you have

recognized

the importance of I&C and setpoint

programs,

and that you have

taken positive steps

in improving your programs

by initiating a guality

Engineering technical

assessment

and

by taking appropriate corrective action.

The team also noted that the Palo Verde Business

Plan includes the support

required to continue your efforts within these

areas.

We consider your self-

assessment

efforts, your commitments to resolving identified problems,

and

your program improvement plans to be positive.

During the period of April 27 June

5,

1992, the

NRC conducted

a system-based

instrumentation

and control

(I&C) inspection of activities authorized

by NRC

licenses

NPF-41,

NPF-51,

and NPF-74.

The inspection

team included

M. Royack,

J. Sloan,

F.

Gee,

D. Acker, T. Sundsmo,

L. Coblentz,

and L. Tran of this

office, and L. King of the

NRC Region II office.

The team held

a discussion

of our findings with members of your staff at the conclusion of the inspection

However,

two of your activities appeared

to be in violation of NRC

requirements,

as specified in the enclosed

Notice of. Violation (Notice).

The

first violation involved failure to evaluate

and take adequate

corrective

actions for safety-related

and important to safet'y instruments routinely found

to be out of tolerance,

including the pressurizer

narrow range pressure

transmitters

and the emergency diesel

generator

(EDG) low lube oil pressure

trip switches.

The second violation involved failure of personnel

to follow

procedures

when performing instrumentation calibrations.

Additionally, one

deviation was identified involving operation of the

EDG air start

system at

a

lower pressure

than

used during system capacity tests

performed to demonstrate

compliance with Palo Verde Updated Safety Analysis design

assumptions.

Based

on the findings of inadequate

corrective actions in two different areas,

and the apparent

acceptance

of IKC technician supervisors that "skill-of-the-

craft" judgment

may supersede

procedural

requirements

in use of HLTE, we are

concerned

(1) that similar cases

of inadequate

engineering

evaluations

and

corrective actions for instruments

found out of tolerance

could be prevalent

throughout the units,

and

(2) that the

I&C technicians'ailure

to follow

procedural

requirements for HETE usage

could result in non-conservative

calibration of safety-related

instrumentation.

In your response

to the Notice

of Violation, we request that you include

an assessment

of these specific

concerns.

In accordance with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"

a copy

of this letter and the enclosures will be placed in the

NRC Public Document

Room.

The response

directed

by this letter and the enclosed

Notice are not subject

to the clearance

procedures

of the Office of Hanagement

and Budget
