NOTICE OF VIOLATION

NRC INSPECTION REPORT

NO. 50-528/529/530/92-17

This refers to the inspection

conducted

by D. Coe,

D. Kirsch, F. Ringwald, J.

Sloan,

and L. Tran of this office, on April 12 through.Hay 30,

1992.

The

inspection

included

a review of activities authorized for your Palo Verde

facility.

At the conclusion of the inspection,

the findings were discussed

with those

members of your staff identified in the enclosed report.

Areas examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements.

The violation is based

on your

mechanical

thread

engagement

program requirements

not being met for several

safety

system valves.

Although your subsequent

analysis

has

so far shown

adequate

strength

in these

cases,

our concern is over the apparent

widespread

nature of these deviations

from program requirements.

Also discussed

during

the inspection

were your activities related to the misinstallation of a check

valve in the high pressure

injection system in August 1989.

This issue will

be the subject of a special

inspection report (50-528/92-23).

In our review of feedwater isolation valve'failures,

we noted that there

have

been several

0-ring failures asociated

with the solenoid valves in the

hydraulic control system since

1991.

The most recent 0-ring failure occurred

on June 9,

1992.

While a design

change

has

been proposed, it appears

that

further actions

are necessary

to evaluate

the latest failure and the impact of

potential

leakage

on the safety function of the feedwater isolation valves.

Please

provide

a description of the actions

being taken to resolve this issue.

In accordance

with 10 CFR Part 2.790 of the NRC's "Rules 'of Practice,"

a copy

of this letter

and the enclosures will be placed in the

NRC Public Document
