This refers to the inspection
conducted
by D. Coe,

and L. Tran of this office, on March 1,

The inspection
included
a Ieview of activities
authorized for your Palo Verde facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the
enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The one violation involved a radiation area which was not
posted
as required in Unit 1.
Because
our further review indicated that this
was
an isolated condition,
and your corrective actions
were prompt
and
adequate,
a response
to this violation is not required.
Your attention is directed to paragraph
7 of the enclosed
report.
This
paragraph
discIIsses electrical distribution and balance-of-plant
equipment
failures which caused
a reactor trip in Unit 2 or subsequently
contributed to
significant water
hammers
in the feedwater
and condensate
systems.
In one of
the cases,
the equipment
problem had
been previously recognized
but not
sufficiently addressed
to preclude
the event.
Additional licensee
management
attention
appears
necessary
to improve equipment
performance
in these
areas
to
reduce challenges
to the plant protection
systems.
Paragraph
5 of the enclosed
report discusses
an event in Unit 2 when work was
performed
on the condensate
system which nearly resulted
in a plant transient.
This work is normally scheduled
to be performed with the unit shutdown,
but
was performed without adequate
precautions
to preclude
a pump trip during

power operations.
This issue is of concern in light of other recent
events
where
weak work control
was evident.
In accordance
with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"
a copy
of this letter and the enclosures will be placed in the
NRC Public Document
