This refers to the inspection
conducted
by D. Coe,
L. Coblentz,
F. Ringwald,
J. Sloan,
W. Ang,
and L. Tran of this office,
on January
26 through February
29,
1992.
The inspection
included
a review of activities authorized for your
Palo Verde facility.
At the conclusion of the inspection,
the findings were
discussed
with those
members of your staff identified in the enclosed report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
certain of your activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
Two cited violations involved 'failure to comply with your
procedures.
In one instance,
the manual withdrawal of fixed incore nuclear
instrumentation
occurred outside the scope of the work instructions.
This
violation is of particular
note in light of the procedural
adherence
problems
during the recent Unit 2 refueling.
In the second
instance,
inattention to
detail during the performance of a surveillance test resulted in a reactor
trip setpoint for one reactor protection
channel
being left below .the minimum
allowed value.
In your response
to this violation, please
provide your
assessment
of the need for independent verification of the reactor trip
'setpoints
adjusted
during surveillance testing.
A third violation was associated
with a failure to ensure
the quality of
gas
used in the calibration of the containment
hydrogen monitors.
Paragraph
8 of the enclosed
report discusses
troubleshooting
performed
on an
essential
spray
pond
pump breaker which had failed to close.
We concluded
that the maintenance
troubleshooting
work was not well planned.
Additionally,
our reviews of previous failures of similar breakers
indicates that the root
cause of failure was not identified for three of five previous failures.

These observations
take
on added significance in light of the recent event at
Unit 3 regarding the failure of a reactor trip breaker.
Paragraph
14 of the enclosed
report discusses
your guality Assurance
audit
program.
Although we found that audits
are generally
good and are performance
based, it appears that the audit findings are not always considered
as
indicators of programmatic problems.
For example,
your audits identified
problems with the delineation of senior reactor operator responsibilities
and
with craft personnel
working outside their work instructions.
These
issues
were not thoroughly addressed
and later contributed to escalated
enforcement
actions.
You -are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should
document the specific actions taken
and
any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"
a copy
of this letter and the enclosures will be placed in the
NRC Public Document
, Room.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
